Christmas is almost here. What a year it's been!! With new services at the clinic and several friendly new faces joining our team, 2018 has been busy for us all.

Continuing things in the same fashion, we're pleased to announce the launch of our new Shockwave Therapy service. 

Shockwave therapy, also known as Extracorporeal Shock Wave Therapy (ESWT), is a clinically proven and highly effective treatment for chronic soft tissue injuries and certain bone conditions.

An extracorporeal shockwave is defined as a non-invasive, non-electrical high energy sound wave that passes through the body via a hand-held probe. The shock waves produce a rapid increase in blood circulation to the target area and will break down fibrous scar tissue which builds up over time, especially with chronic conditions.

 
An Introduction to Tendinopathy and Loading
Tendinopathy refers to a disease of a tendon. The clinical presentation includes tenderness on palpation and pain, often when exercising or with movement.

Several terms have evolved in the medical terminology to refer to injuries that cause tendon pain:
  • Tendinitis - acute tendon injury accompanied by inflammation
  • Tendinosis - chronic tendon injury with degeneration at the cellular level and no inflammation 
  • Paratenonitis - Inflammation of the outer layer of the tendon (paratenon) alone, whether or not the paratenon is lined by synovium
  • Paratenonitis with tendinosis - Paratenonitis associated with intratendinous degeneration
  • Tendinopathy - chronic tendon injury with no implication about etiology
Common symptoms of tendinopathy are:
  • localised burning pain and swelling around the tendon
  • pain that gets worse during and after activity
  • stiffness in the joint
  • restricted joint movement
  • pain that persists for several months

Can Tendinopathies Be Treated? And If So, How?
Injury rehabilitation of tendinopathies must utilise a combination of education, tendon load management, kinetic chain strengthening and a graded return to running/sport.

Education
This ensures YOU know the details of your tendinopathy – what it is, why you have it, the timings and essential information involved in YOUR personal recovery.

Load Modification
RELATIVE rest is essential in order to reduce the aggravating factors inflaming the tendon; thus, settling the cell/immune response. Complete rest is NOT indicated as it decreases the ability of the tendon to take load. Relative rest involves a reduction (not cessation!) of tendon aggravating activities or sports.

Tendons take a long time to heal because the blood supply to tendons is typically low. Tendinopathies may take 3 to 6 months to heal, but physiotherapy may improve the outlook.

At Therapeutics your Physiotherapist may recommend Shockwave therapy to promote the healing of the tendinopathy in conjunction with a loading program.

I qualified in 2000 from Brunel university. I worked in and around London (barring a couple of spells in Australia) from then until we moved down to Southampton in 2009. I worked at several teaching hospitals and spent my spare time playing rugby for a team called Rosslyn Park.

Since moving to Southampton I and my family have taken up sailing at our local club in Warsash and we all go to CrossFit Fareham. Both my kids are involved with the junior section of Trojans and I help out with the under 8’s.

My working life for the last 8 and a half years has been at Therapeutics and during that time I have worked for Hampshire Cricket, GB sailing, Hampshire U16 golf and Eastleigh Rugby. We also look after the physical health of the local TASS athletes based at the university.

We are always updating and renewing our professional skills and knowledge - the last course I went on was related to the management of tendinopathies. This had a great impact on my and the clinics management of these tricky and awkward-to-manage problems.

I like to use a functional approach with my clients when assessing their issues as I believe it enables the effective management both in the short and long term and also forms a significant part of injury prevention in the future.
