CVS Partners With 4 Major Providers for Care Coordination, Record-Sharing

Through a new partnership, patients of four major providers — ProHealth Physicians in Farmington, Conn., Texas Health Resources in Arlington, Palmetto Health in Columbia, S.C., and the Baton Rouge (La.) Clinic — will be able to access care management and wellness programs at CVS' MinuteClinics.

The partnership will rely on electronic medical record integration to allow CVS clinicians to add clinic visit or medication information to patient records. The goal is to improve care coordination and foster collaboration between the traditional providers and the retail clinics on promoting long-term wellness.

"Many patients rely on their local pharmacist for information and support regarding their prescription medications and these affiliations enable CVS pharmacists to help improve affiliated health care providers' patient health outcomes through better medication adherence," said Troyen A. Brennan, MD, CMO of CVS, in a news release. "MinuteClinic also plays an important role by providing patients with timely, affordable and high-quality walk-in healthcare."
